Dear Parents/Guardians,
Throughout the school year, some middle school students have expressed a desire to "Walk the Ave" after school, meaning they wish to walk up Germantown Ave. into Chestnut Hill. To ensure clear communication and the safety of all students, we require this permission slip to be completed if you wish to allow your child to do so.
Purpose: This permission slip is to confirm that you grant your child permission to "Walk the Ave" after school throughout the school year. This form only needs to be completed once. Once on file, we will trust that your child has permission to walk the Ave whenever they inform their teacher so please discuss your expectations around this with your child. We will verify who has this form on file and only grant permission to those students. Students without this form on file will be sent to the bus or directed to carline for pick-up.
Responsibilities: By completing this form, you acknowledge that you take full responsibility for your child’s safety once they leave school grounds. We strongly encourage you to discuss proper safety guidelines and procedures with your child, including:
- Crossing streets safely, especially at the crossing guard located in front of Norwood Campus on Germantown Ave. and at designated crosswalks.
- Being aware of their surroundings at all times.
Communication: We also encourage ongoing communication between you and your child regarding their plans to "Walk the Ave" on any given day. This ensures that you are aware of when your child will be walking home and can discuss any concerns or arrangements in advance.
Action: Upon receipt of this completed form, we will inform the appropriate school personnel so that your child may be released to walk home each day.
Please fill out the information below and return this form to the school office.
Thank you for your cooperation in ensuring the safety and well-being of our students.
Sincerely,
Kathryn Henry
Principal
