By completing this form, you give permission for your child to walk home from school/practices/games each day. Please understand that by granting this permission, you take full responsibility for your child’s safety once they leave school grounds.
Please review with your child the proper safety guidelines and procedures for walking home, including crossing streets safely (at crossing guard in front of Norwood Campus on Germantown Ave. at dismissal and at crosswalks) and being aware of their surroundings.
Upon receipt of this form, we will inform the appropriate school personnel of this permission so that your child may be released to walk home each day.
